Drivers Physical Form

In preparation for your physical, please download and fill out this document prior to arriving for your scheduled appointment. Please also bring any of your medications (actual prescriptions) and any clearance letters already provided by the prescribing provider. Failure to supply clearance letters will result in a shortened medical card timeline and additional fees for reprocessing your information once the clearance letters are received.
